treatment. Instead of paying separately for every appointment, membership gives patients a predictable monthly cost while supporting continuity of care.

This membership program is not intended for individuals seeking continuation or management of long-term benzodiazepine therapy. Patients who have been taking benzodiazepines continuously for more than six months may not be appropriate for the membership program. Acceptance is subject to the provider’s clinical, medical history review, and applicable prescribing laws.
Annual laboratory testing may be required as part of ongoing psychiatric treatment. Specific laboratory testing will depend upon medications prescribed, medical history, symptoms, and clinical indication. Patients are responsible for completing requested laboratory testing within the timeframe recommended by the provider.
Patients who are prescribed stimulant medications may be required to obtain an EKG and/or other appropriate medical evaluation or monitoring based on age, medical history, cardiovascular risk factors, symptoms, medications, and clinical judgment. Stimulant prescribing is not guaranteed by enrollment in the membership program
Membership provides access to psychiatric evaluation and treatment, it does not guarantee that any particular medication, controlled substance, stimulant, benzodiazepine, or other treatment will be
prescribed.

Patients may still choose to participate as self-pay patients if permitted under applicable payer and practice requirements. Membership fees are generally not submitted to insurance unless specifically stated by the practice.
Yes. The comprehensive initial psychiatric evaluation is included with the $149/month Standard Membership rather than requiring a separate $200-$250 upfront evaluation fee.
Patients who are beginning treatment or undergoing medication adjustments will generally be seen approximately monthly.
The Maintenance Membership is intended for established, clinically stable patients who have had no medication changes for at least six months. New patients will generally begin with the Standard Membership unless otherwise determined by the provider.
No. Prescription medication costs, laboratory testing, EKGs, imaging, medical appointments, pharmacy costs, and other services provided outside Mental Odyssey Psychiatry are separate from the membership fee.
Stimulants may be considered when clinically appropriate and permitted under applicable prescribing laws and practice policies.

Membership Cancellation Policy
Memberships may be canceled at any time by providing written notice to Mental Odyssey Psychiatry through the patient portal or other approved communication method.
Membership fees are nonrefundable and will not be prorated. If a membership is canceled during the middle of a paid membership month, the patient will continue to receive membership benefits through the final day of that paid billing period. No partial refund or credit will be issued for unused services.
Cancellation requests should be submitted as soon as possible and before the next scheduled billing date to prevent an additional monthly charge. Cancellation requests received after a payment has been processed will apply to the following billing period, and the processed payment will not be refunded. Cancellation of the membership constitutes termination of the patient-provider relationship with Mental Odyssey Psychiatry, effective on the final day of the paid membership period. After that date, the practice will no longer be responsible for appointments, medication refills, prescription renewals, laboratory monitoring, prior authorizations, clinical advice, or other ongoing treatment needs. Patients are responsible for establishing care with another qualified healthcare provider before the membership ends to avoid interruptions in treatment or medication access. When clinically appropriate and permitted by law, Mental Odyssey Psychiatry will provide reasonable assistance with the transfer of medical records upon receipt of a valid written authorization. Cancellation of the membership does not relieve the patient of responsibility for any outstanding balance or charges incurred before the termination date.
